Deficient Fluid Volume r/t Active Fluid Volume Loss 

 

Definition:

This means decreased intravascular, interstitial, and/or intracellular fluid, referring to dehydration, water loss alone, without change in sodium level.

 

Characteristics:

Change in mental state; decreased blood pressure, pulse pressure and pulse volume; decreased skin and tongue turgor; decreased urine output; decreased venous filling; dry mucous membranes; dry skin; elevated hematocrit; increased body temperature; increased pulse rate; increased urine concentration; sudden weight loss (except third spacing); thirst; weakness

 

Nursing Outcomes:

  • Fluid balance, hydration, nutritional status; food and fluid intake

 

Client Outcomes:

  • Maintain urine output of 0.5mL/kg/hour

  • Maintain normal blood pressure, pulse, and body temperature

  • Maintain elastic skin turgor; moist tongue and mucous membranes; and orientation to person place , and time

  • Explain measures that can be taken to treat or prevent fluid volume loss

  • Describe symptoms that indicate the need to consult with health care provider

 

Intervention:

  • Fluid management, hypovolemia management, shock management: Volume
